COVID Information & Waiver
Proof of up-to-date vaccinations (primary series of WHO recognized covid-19 vaccines and any applicable boosters) at least 14 days prior, along with photo I.D., are required to be shown at the beginning of the first class for all vaccine eligible students ages 5 and up. Instructors are fully vaccinated.
This is an indoor class and masks are optional. However, some external studios where we hold our classes may require masking. We have erred on the side of caution during the pandemic, and will continue to do so.
I acknowledge the contagious nature of the COVID-19 virus, and that the CDC and many other public health authorities still recommend practicing social distancing and wearing face coverings. I further acknowledge that Mona Khan Company or any associated entity has put in place preventative measures to reduce the spread of the COVID-19 virus, to the best of their abilities.
I further acknowledge that Mona Khan Company or any associated entity can not guarantee that I will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, Mona Khan Company staff, and other patrons and their families.
I voluntarily seek services provided by Mona Khan Company or any associated entity and acknowledge that I am increasing my risk to exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all set procedures to reduce the spread while attending my classes.
I attest that:
- I am not experiencing any symptoms of illness such as cough, shortness of breath, difficulty breathing, fever, chills, muscle pain, headache, sore throat, or new loss of taste or smell.
- I do not believe I have been exposed to someone with a suspected and/or confirmed case of COVID-19.
- I have not been diagnosed with Coronavirus/Covid-19 by state or local public health authorities in the past 10 days.
- I am following all CDC recommended guidelines as much as possible, including limiting any purposeful exposure to COVID-19.
I hereby release and agree to hold Mona Khan Company and any related entity harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act from Mona Khan Company or associated entity, or that may otherwise arise in any way in connection with any services received from Mona Khan Company or any associated entity. I understand that this release discharges Mona Khan Company or any associated entity from any liability or claim that I, my heirs, or any personal representatives may have against Mona Khan Company with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Mona Khan Company or any associated entity. This liability waiver and release extends to Mona Khan Company together with all owners, partners, instructors, and staff.
